If you answered "Yes" to question 10, please complete Questions 11 to 23.  If you answered "No," skip to question # 24.

Your answers help us provide you with targeted feedback about adoption.

11.  Are you asthmatic?

12.   Do you use an inhaler?             If so, how many times a day?

13.   How quickly after meeting a cat do you react?

14.   What are your specific cat allergy reaction symptoms?

15.   On a scale of 1 to 4, with four being most severe, and one being no reaction, how would you rate your allergic reaction?

16.   Are you multiply allergic (dogs, dust, mold, pollens, etc)?

17.  Are you willing to learn about housecleaning and kitten care strategies to keep allergens low and more importantly to commit to implementing them faithfully? Your long term success and overall comfort will be immeasurably improved with allergy-reduction strategies in place. _____

18. What type of heating/cooling system do you have in your home:

Radiator____ forced air ___ baseboard electric___ Wood stove _____if other, please specify___________

19.  Do you understand that in order to adopt as an allergic home, you will need a "Safe Home Contact"* to co-sign your contract specifying that they will provide a permanent home for your kitten if allergies ever arise? We do not refund or rehome allergy placements. You are responsible for identifying and securing a permanent home for your kitten before you adopt. ______

* Your "Safe Home" is your friend or family member who is non cat-allergic and agrees to provide a permanent home for you if you have any allergy reactions to your kitten /or cat post-adoption.

20. Do you understand that an Allergy Adoption with Forest Wind is not a "try it and see" situation? You are making a commitment to modifying your lifestyle (housekeeping/ home decorating) and the care and feeding of your kitten (top quality diet + weekly bathes + kitten bedroom). Without your partnership, your adoption may not be successful. Siberian cats are not "magic bullets." They are a breed that produces less allergen. Your part is to ensure that what is produced doesn't hang around to make you reactive.  Yes, I understand I need to make a commitment to allergy friendly house and cat keeping strategies in order to ensure a successful adoption: ______

21. Do you understand kittens cannot be tested for FELD-1 levels? Cats must be at least age one, and preferably aged two, to give the most accurate feld-1 level test results. Therefore, you will not know your kitten's allergen level when you adopt. _______

22. Do you understand that there is a 25% chance that two low allergen Siberian parent can give birth to a normal allergen kitten? Statistically, there is a 75%  chance for each kitten in a two low allergen parent litter to also be low. ______

23. Are you willing to keep your kitten out of your bedroom, and to provide a "kitten bedroom" for her in order to maximize your chance of a successful adoption? __________

Thank you for completing the allergy survey portion of the inquiry form :)
